1. Registration & Fees

- Teams must register by deadline on the tournament tab with a fee of $450 for men's and coed $325 for women.

- Full payment must be made at the time of registration—no refunds unless the tournament is canceled by the facility.

- Each team must submit a finalized roster with the maximum of 10 players.

2. Eligibility & Team Composition

- All players must be **18 years or older** and sign a liability waiver before participation.

- Teams must have at least 4 players present to start a match.

- Substitutions are allowed following tournament rules.

3. Tournament Format & Scheduling

- Teams must arrive 20 min before kickoff for check-in.

4. Conduct & Sportsmanship

- Players must **respect referees, opponents, and facility staff** at all times.

- **No fighting, abusive language, or unsportsmanlike behavior** will be tolerated.

- The facility reserves the right to **remove players or teams** for misconduct without a refund.

5. Game Rules & Referee Decisions

- Games will follow Momentum NWA rules and regulations.

- Referee decisions are **final**, and disputes will be handled by tournament organizers.

- Yellow and red cards will be issued in accordance with tournament regulations, with disciplinary action determined by event officials.

6. Liability & Insurance

- The facility is **not responsible** for injuries, lost items, or personal property damage.

- All participants must sign a **waiver** acknowledging their own risk in play.

- Teams may be required to provide proof of insurance.

7. Equipment & Attire**

- Players must wear matching t-shirts but matching uniforms are preferred and use approved **indoor soccer shoes** (no cleats).

- Shin guards are **mandatory** for all participants.

- The facility provides game balls—personal balls must be kept off the field unless approved.

8. Facility Rules & Cleanup

- No food or drinks (except water) are allowed on the playing surface.

- Teams must **clean up** their bench areas after each match.

- Any damage to facility equipment or property must be reported immediately and may incur a charge.

9. Prizes & Awards

- Tournament winners will receive medals.

- The facility reserves the right to modify prize details if necessary.

10. Agreement & Acknowledgment

By registering, all teams and players agree to follow these terms and conditions.
