Youth Indoor Soccer League – Terms & Conditions 

1. Registration & Fees 

- Each team must complete registration and pay the league fee of $400 before the season begins.  

- All players must have **parental consent** and submit signed waivers before participation.  

- Registration fees are **non-refundable**, except in cases where the league is canceled by the facility.  

2. Player Eligibility*

- Players must be between 5-17 years old to participate  

- Teams must have a minimum of 5 players on their roster.  

- Substitutions are allowed following league rules.  

3. Scheduling & Attendance 

- Games will take place see schedule tab.  

- Teams must check in 20 minutes before kickoff.  

- A team forfeiting 3 games may be removed from the league without a refund.  

4. Safety & Conduct

- **No rough play, aggressive behavior, or bullying** will be tolerated.  

- Coaches and spectators must **respect referees and staff** at all times.  

- The facility reserves the right to **suspend or remove** individuals or teams for misconduct.  

5. League Rules & Gameplay  

- Games will follow Momentum NWA rules and regulations.  

- Referee decisions are **final**—no protests allowed.  

- Players receiving 3 red cards may face additional disciplinary action.  

6. Liability & Insurance

- The facility is not responsible for injuries, lost items, or personal property damage.  

- Players participate at their own risk and must have signed **liability waivers** before competing.  

- Teams may be required to provide proof of insurance.  

7. Equipment & Uniforms 

- Teams must wear **matching jerseys** with numbers visible.  

- **Shin guards are mandatory** for all players.  

- Only **approved indoor soccer footwear** is allowed—no cleats.  

8. Facility Rules & Cleanup

- No food or drinks (except water) are allowed on the field.  

- Teams must **clean up their bench area** after every game.  

- Damage to the facility or equipment must be reported immediately.  

*9. Parent & Coach Responsibilities

- **At least one adult coach** must be present on the bench during games.  

- Parents are expected to **model good sportsmanship** and follow facility guidelines.  

- Any disruptive behavior from adults may result in removal from the premises.  

10. Agreement & Acknowledgment

By registering, all teams, players, parents, and coaches agree to follow these terms and conditions.
